Cause Description:
Community Bridges is a not-for-profit agency established in 1982 that has served as a single source for support to people with disabilities and their families who live in Merrimack County. Through a relationship with the State of New Hampshire as the area agency for planning and provision of developmental services in the region, it offers a wide range of life-long supports including case management, service development, service contract management, benefits coordination, family support, infant/toddler services, and family respite services. Individuals in need of long-term care receive help through service brokerage to access a wide range of agencies specializing in supporting people with mental retardation to help them find a good home or a good job and keep it. Early Intervention provides identification, treatment and prevention of developmental delays for those infants and toddlers who have a delay in any area of development or are at risk for disabilities.
